To begin the enrolment process, we ask that you complete and submit an Application for Enrolment. Before you begin your application, please take note of the following required documents:
- Child’s birth certificate
- Recent school or preschool reports (if applicable)
- Relevant Court Orders or Parenting Plans (if applicable)
- Learning Assessments / Diagnostic Reports (if applicable)
- Medical Plans / Medication Care Plans (if applicable)
- Visa/Passport Documents (if applicable)

Download the Application for Enrolment form below and submit, along with the applicable documents listed above, to info@smm.catholic.edu.au or drop them into the Front Office at 82 Fairfield Road, Elizabeth Grove.
Once we receive your Enrolment paperwork, our leadership team will review and you will be contact to book an Enrolment Discussion.
